Abstract

There is clear evidence that laser treatment is effective and universally agreed for the treatment of proliferative diabetic retinopathy (DR). The evidence that intensive blood glucose control reduces the risk of new‐onset DR and slows the progression of existing diabetic retinopathy comes from the Diabetes Control and Complications Trial (DCCT). There is widespread agreement that digital photography is the best method of screening for sight‐threatening DR. This chapter presents the reasons for digital photography being the preferred method. Widefield scanning laser ophthalmoscopes have been proposed for diabetic retinopathy screening but have not as yet demonstrated the same level of resolution as digital cameras. Studies of mydriatic digital photography against a recognised reference standard of seven‐field stereo photography and an ophthalmologist using slit‐lamp biomicroscopy have shown showed consistently good results. Computer algorithms can detect microaneurysms and some other features of diabetic retinopathy to a high degree of accuracy.
