Abstract

Optimum control of glycaemia, hypertension and lipids are recommended for all patients with diabetes. The earliest signs of mild non‐proliferative diabetic retinopathy (mild NPDR) or background DR are microaneurysms. A venous loop is an abrupt curving deviation of a vein from its normal path, and the early treatment of diabetic retinopathy study (ETDRS) included a single venous loop in their classification of mild NPDR. This is now also a feature of the UK screening definition of background diabetic retinopathy. Larger venous loops also occur in later ischaemic stages of the disease process. In mild NPDR, retinal haemorrhages are usually small dot haemorrhages or flame‐shaped haemorrhages. Because small retinal haemorrhages can be difficult to differentiate from microaneurysms, they are commonly re‐ferred to as HMa. Flame‐shapedhaemorrhages are present just under the superficial nerve fibre layer. Blot haemorrhages are usually in a deeper retinal layer and denote a sign of ischaemia.
